What is Triamcinolone?

Triamcinolone, pronounced as tri-am-cin-o-lone (try-am-SIN-oh-lone), is a corticosteroid medication used primarily to treat inflammation and itching caused by skin conditions like eczema, psoriasis, and allergic reactions. It’s also available as injections and a nasal spray to treat other kinds of inflammation*.

As a steroid medicine, triamcinolone is used to help treat many different conditions, including eczema, psoriasis, arthritis, allergies, ulcerative colitis, lupus, and breathing disorders. It comes in many formulations, including topical creams, nasal sprays, oral pills and injectables – depending on the condition it’s used to treat*

It works by reducing the activity of the immune system, thereby dampening the inflammatory response. Triamcinolone acetonide cream is available under various brand names such as Kenalog, Triderm, Aristocort, Trianex, Oralone, and others. It is available in various forms:

  • Creams and ointments are used for mild to moderate inflammation of the skin.
  • Lotions are ideal for hairy areas or larger skin surfaces.
  • Gels provide a cooling effect and are suitable for scalp conditions.
  • Solutions are used for scalp conditions like seborrheic dermatitis.
  • Aerosol sprays are convenient for large areas or hard-to-reach spots.
  • Oral paste/gel is used for mouth sores and inflammation.
  • Injections are administered directly into inflamed joints or muscles for localized treatment*.

By reducing inflammation and muting the body's immune response, triamcinolone helps alleviate many inflammatory and auto-immune symptoms. Here’s a summary of some of the most common uses of triamcinolone:

What is topical triamcinolone acetonide used for?

Triamcinolone acetonide is a corticosteroid medication available in various strengths and forms, like creams, ointments, lotions, and gels. It reduces inflammation and suppresses the immune system's response, making it helpful in treating various skin conditions. Common uses include:

  • Eczema: Triamcinolone alleviates redness, itchiness, and inflammation associated with eczema flare-ups (strength examples: 0.1%, 0.05%, 1.0%).
  • Poison ivy: The medication relieves swelling, itching, and discomfort caused by poison ivy rash (strength examples: 0.1%, 1.0%).
  • Psoriasis: The cream manages redness, scaling, and inflammation associated with psoriasis plaques (strength examples: 0.1%, 0.05%).
  • Diaper rash, jock itch, vulvar itching: Triamcinolone soothes inflammation and itchiness in sensitive areas (strength examples: 0.1%, 1.0%).
  • Contact dermatitis and hives: It relieves symptoms such as itchiness, redness, and swelling caused by allergic reactions (strength examples: 0.1%, 1.0%).
  • Sunburn: The medication offers temporary relief from redness, pain, and inflammation caused by mild sunburn (strength examples: 0.1%, 1.0%).

Triamcinolone acetonide dental paste

Triamcinolone acetonide dental paste is a topical corticosteroid used to treat oral inflammation caused by conditions such as canker sores, oral thrush, oral lichen planus, gingivitis, aphthous stomatitis, and post-surgical inflammation.1 This drug is a dental paste that adheres to the inside of the mouth, gums, and cheeks. It is classified as a medium-strength corticosteroid, and it suppresses the immune system's response and reduces inflammation, providing rapid relief from oral inflammatory conditions. This paste allows targeted treatment, maximizing its effects while minimizing body-wide absorption and reducing itching, swelling, and pain associated with mouth sores.

Less common uses of this treatment include:

  • Managing conditions such as keloids, alopecia areata, and vitiligo
  • Addressing joint pain
  • Addressing seborrheic dermatitis and plantar fasciitis depending on the severity and alternative treatment options
  • Treating cystic acne in specific cases alongside other acne treatments

There are several oral inflammation alternative treatments, including antifungal medications such as nystatin or clotrimazole for oral thrush, antiseptic agents for relief, medicated mouthwashes with corticosteroids, and even dietary changes such as avoiding sugary foods to manage fungal overgrowth. Your doctor will direct you on how to use this paste, but general instructions may include simply pressing or dab a small amount onto the affected area until a smooth film develops, usually two to three times daily. Avoid covering or bandaging the area unless advised by your doctor.

Note that:

  • Triamcinolone acetonide dental paste requires a prescription from a doctor or dentist.
  • It addresses symptoms but does not cure any underlying conditions such as oral lichen planus.
  • It has potential side effects such as thrush worsening, fungal overgrowth, and local irritation.
  • It is only recommended for short-term use (up to three weeks) to minimize side effects.
  • It is not suitable for people with certain underlying conditions or allergies.

How to take triamcinolone

Take triamcinolone exactly as prescribed by your doctor. Your doctor might adjust the dose to ensure optimal results.

Take triamcinolone with food to avoid stomach upset, and inform your healthcare provider of any medical situations or unusual stress. To prevent any withdrawal symptoms, consult your doctor before you stop using the medication.

If you miss a dose of triamcinolone, take it as soon as you remember; however, if it's almost time for the next dose, skip it and don’t take any extra medicine to compensate for it.

It is important to avoid double-dosing because prolonged use of high steroid doses leads to various symptoms such as easy bruising, changes in body fat distribution, thinning skin, menstrual problems, low sex drive, impotence, and increased acne or facial hair.

Triamcinolone acetonide side effects

Topical steroids like triamcinolone reduce inflammation effectively, but this can also lead to thinning of the skin if used longer than directed, so be sure to talk to your doctor about duration of use. This medication may also rarely cause itching, burning, dryness, or irritation when applied to the skin. However, these effects should clear up within a few days as the body adjusts to the medication, but if they persist, call your doctor.

Contact your doctor immediately if you encounter any of these severe side effects2:

  • Swelling
  • Vision problems
  • Rapid weight gain
  • Severe depression
  • Seizures
  • Unusual behavior or thoughts
  • Shortness of breath
  • Coughing up blood
  • Bloody or tarry stools
  • Pancreatitis (severe upper stomach, pain in the back, vomiting, nausea, fast heart rate)
  • Low potassium (irregular heart rate, confusion, increased urination, extreme thirst, limp feeling, muscle weakness, or leg discomfort)
  • Dangerously high blood pressure (severe headache, blurred vision, buzzing in the ears, anxiety, confusion, chest pain, shortness of breath, irregular heartbeats, seizures).

Less severe side effects of triamcinolone may include:

  • Acne, dry skin, thinning of the skin, bruising, or discoloration
  • Delayed wound healing
  • Increased sweating
  • Nausea, stomach pain, or bloating
  • Muscle weakness
  • Changes in body fat distribution, particularly in the arms, legs, face, neck, breasts, and waist

Triamcinolone alternative prescriptions

Some people may not respond well to triamcinolone or experience side effects that make it difficult for them to continue using it. In such cases, your doctor may prescribe alternative drugs. Some common alternatives include prednisone, methlyprednisone, dexamethasone, and metamethasone. These drugs all work to reduce inflammation by suppressing the immune system.

Triamcinolone vs. hydrocortisone

Triamcinolone acetonide and hydrocortisone are both topical corticosteroids but with different potency levels. Hydrocortisone is considered a low to medium-potency steroid, while triamcinolone falls into the medium-to-high potency range. Also, triamcinolone is a prescription drug, while hydrocortisone is available OTC.

They have distinct administration methods, resulting in different absorption patterns and drug interactions. Triamcinolone acetonide cream may interact with specific cancer chemotherapy agents and other topical medications. Precautions may apply to people with immune disorders, circulatory disorders, or eye conditions.

Triamcinolone vs. mometasone

Mometasone topical is another potent steroid that relieves itching, swelling, redness, or other discomfort caused by certain skin conditions, atopic dermatitis, psoriasis, and eczema.3 It is a prescription medicine and should not be used on children under two years. Inform your doctor if you have diabetes, skin infection, breastfeeding, have unusual allergies, and if you're using other medications to avoid the possibility of drug interaction.

Triamcinolone vs. clobetasol

Clobetasol is a highly potent corticosteroid used to treat various skin and scalp conditions, such as plaque psoriasis, eczema, and lichen sclerosus.4 It helps reduce swelling, inflammation, itching, and redness by activating natural substances in the skin.

It comes in different forms: solution, gel, cream, ointment, foam, or spray. Be cautious as you apply it on open wounds or large areas of the skin. This medication is typically used for short-term treatment. It may cause different side effects, such as worsened skin conditions, allergic reactions, temporary hair loss, and high blood sugar, and absorption into the body.5 If these symptoms worsen, consult your doctor.

Triamcinolone topical vs. desonide topical

Desonide helps reduce itching and inflammation caused by eczema and other skin conditions that respond to steroid medication.6 It can cause side effects such as skin irritation, swelling, or dryness.  If you experience severe side effects such as blurry vision or high blood pressure, call your doctor immediately.

Apply a thin film of the cream/ointment to the affected areas two to four times daily, depending on the severity of the condition, for two weeks. It is essential to discontinue the treatment once control is achieved. If no improvement is observed within two weeks, a reassessment of the diagnosis may be necessary.

Oral alternative: prednisolone soluble tablets

Prednisolone is a synthetic corticosteroid used to treat conditions such as immune system disorders, arthritis, skin and eye conditions, and allergies.7 It reduces the immune system's response to alleviate swelling, pain, and allergic reactions. Follow your doctor's instructions if you are taking prednisolone orally, take it with milk or food to prevent stomach upset, and use a particular measuring device to ensure accurate dosing.

Liquid prednisolone is available in different brands, forms, and strengths. Always read the dosing instructions carefully, as the amount of prednisolone may differ. Also, inform your doctor before discontinuing the medication, as sudden withdrawal may worsen certain conditions.

Drug interactions

Triamcinolone can interact with several drugs.8 The following is a partial list; please inform your doctor if you are using any of these:

  • Aspirin (taken daily or in high doses)
  • Cyclosporine (Gengraf, Neoral, Sandimmune)
  • Rifampin (Rifadin, Rifater, Rifamate, Rimactane)
  • Seizure medications such as phenytoin (Dilantin) or phenobarbital (Luminal, Solfoton)

Remember, this list is not exhaustive, and other drugs may also interact with triamcinolone. Be sure to inform your doctor about all your current medications, including prescription, over-the-counter, vitamin, and herbal products. It is essential to consult your doctor before starting any new medication.

Triamcinolone FAQs

Does triamcinolone acetonide cream lighten skin?

Triamcinolone acetonide cream lightens skin, but it's not intended for that purpose and should not be used for skin lightening. Corticosteroids such as triamcinolone suppress melanin production, which leads to uneven and unpredictable lightning effects.

Remember that the risks of using triamcinolone for skin lightening outweigh any potential benefits. Risks include irritation, acne, stretch marks, rebound darkening, and skin thinning. Instead, use safer alternatives such as chemical peels, cosmeceuticals, and laser treatments.

Is triamcinolone acetonide a steroid?

Yes, triamcinolone acetonide is a steroid. It belongs to a class of steroids called corticosteroids (glucocorticoids). It is used to treat various conditions such as skin conditions (eczema, psoriasis, allergic reactions, dermatitis), joint pain and inflammation (arthritis, bursitis, tendinitis), and respiratory conditions (asthma, allergic rhinitis).

However, it's essential to use it as prescribed by your doctor due to potential side effects like skin thinning, increased infection risk, weight gain, mood swings, and bone loss. Always use them as your doctor prescribes, and be aware of these potential side effects.

Can nystatin and triamcinolone acetonide be used together?

Nystatin and triamcinolone acetonide are sometimes used to treat fungal skin infections and cutaneous candidiasis. Nystatin kills fungal cells, while triamcinolone acetonide reduces itching and inflammation. However, this combination is unsuitable for all types of fungal infections and should only be used under the guidance of a doctor.

Is triamcinolone acetonide an antifungal cream?

Triamcinolone acetonide is not an antifungal cream. It belongs to the corticosteroid class of drugs and reduces inflammation and itching. While it relieves pain from itching, it doesn't target the fungus.

On the other hand, antifungal medications, such as clotrimazole and nystatin, inhibit or directly kill the growth of fungi. Sometimes, if you have significant inflammation or difficult-to-diagnose conditions, your doctor might prescribe a combination of triamcinolone acetonide and antifungal medication.

How long does triamcinolone cream stay in your system?

Duration varies depending on skin condition, frequency of use, and individual metabolism.

Generally, the cream primarily affects the applied area, with residual effects lasting several hours to a few days. However, trace amounts of the drug may be detectable in blood or urine for up to a week or two. However, these are general estimates and vary from person to person.

How long can you use triamcinolone acetonide?

Seven days is usually enough to treat symptoms of common skin conditions. If there are no improvements in your symptoms after seven days, inform your healthcare provider. Do not continue using the medicine unless advised by your doctor.
