Weight loss medication FAQs

  • What do doctors normally prescribe for weight loss?

There are a variety of weight loss drugs that are FDA approved for treatment of weight loss, including Saxenda (liraglutide), Xenical (orlistat), Wegovy (semaglutide), Contrave (bupropion-naltrexone), and Plenity. Our board-certified doctors can prescribe weight loss medications, after conducting an initial consultation online and ordering any necessary evaluations to the lab nearest you. After completing any necessary testing, your doctor will discuss the best medication options for you, if you qualify. It is important to note that medication is only effective in combination with healthy lifestyle changes.

  • Who shouldn’t take weight loss medications?

Talk to your doctor about whether it is safe to take weight loss medications if you have any of the following conditions:

    • Kidney disease

    • Heart disease

    • Pancreatitis

    • Thyroid tumors

    • Hypoglycemia

    • High blood pressure

    • Epilepsy

In general, weight loss medications are for patients with a BMI of over 27. If your BMI is less than 27, our doctors can help with diet, exercise, health coaching, and may be able to prescribe weight loss medications such as Topiramate, Wellbutrin, Contrave, or Plenity.

If you have symptoms of an active eating disorder, including bulimia nervosa, anorexia nervosa, or binge eating disorder, our doctors cannot treat you with weight loss medication. However, our doctors are still happy to speak with you about any of your health concerns.

  • How long does it take for weight loss medications to work?

Most patients on weight loss medications can begin losing weight within weeks, when used in combination with a healthy diet and exercise plan. If you are not seeing results within 12 weeks, your doctor may develop a different weight management plan.

  • What should I avoid with weight loss medications?

Weight loss medications can interact with a variety of drugs. These can include, but are not limited to:

    • Alcohol

    • Other diabetes medications

    • Insulin

    • Hormonal birth control

    • Herbal weight loss products or other supplements

    • Vitamin B12 and vitamin D3

    • Gabapentin

  • Is weight loss medication safe for me to take?

Every medication has potential side effects, risks, and benefits. Talk to one of our doctors to discuss the pros and cons of using medications for weight loss for your unique needs.

  • Am I eligible for weight loss medication?

Weight loss medication is not necessarily for everyone. Book a visit with one of our board-certified doctors to determine the optimal weight management plan for your specific needs. This plan may or may not include medication. However, if you are not eligible for medication, our doctors can still help with diet, exercise, and health coaching as part of a customized weight management plan.

  • Can I use insurance for weight loss medication?

Yes, we are in network with most major insurance providers.

However, most branded weight loss medications require prior authorization in order for insurance to cover the cost. Your PlushCare physician and care team will reach out to your insurer on your behalf, and this process can take several business days. If you are denied the prior authorization or do not have health insurance, you may still purchase the medication at the full cash price.

  • How much will weight loss medication cost?

Prices vary depending on the medication type, and whether or not you use insurance to pay for it. Most branded weight loss medications require prior authorization in order for insurance to cover the cost. The good news is that even if you are uninsured, our members save an average of 58% on prescriptions through our member discount card.

  • Is weight loss medication effective?

Weight loss medication has proven to be an effective component in losing and keeping off weight, but it should be complemented by appropriate dietary choices and exercise. Our doctors will work with you to determine if medication is right for you, and they will set realistic expectations based on your personal habits and lifestyle.

  • I have a normal BMI—can I get a prescription for a weight loss drug?

No, we do not treat new patients with normal BMI with weight loss pharmacotherapy, unless the patient shares outside records documenting previous obesity.

  • If I take weight management medications, do I still need to eat healthy and exercise to lose weight?

Yes, weight loss medications are most effective when part of a holistic treatment plan, including long term lifestyle changes. A board-certified doctor will work with you to determine a diet and exercise plan that works for you.

  • Are weight management medications safe?

There are many safe FDA-approved weight loss medications for long-term use. These medications are safe for most people. Our doctors will work with you to evaluate your options and choose a weight loss medication that is safe and effective for you.

  • How much weight can I expect to lose with lifestyle modification and weight management medications?

You can expect to lose about 5-10% of your body weight, compared to your starting weight. This means that if you weighed 200 pounds, you could expect to lose 10-20 pounds, by incorporating lifestyle changes in addition to medication.

  • Once I lose weight, can I stop my weight management medication?

It depends on the individual.  Some patients can effectively discontinue weight management medications when they reach their goal, but others may need to stay on medicine for the long term to avoid relapsing.  Weight loss medication is often meant for long term use. Once you lose weight, you may need to continue the medication to avoid relapse. Your PlushCare doctor can help you make the decision on whether to continue or stop medications.

  • What happens when you stop taking weight loss medications?

When weight loss medications are stopped, many people gain the weight back. Drugs like Wegovy and Ozempic lower a person's appetite to help with weight loss, but experts suggest that once people stop taking these drugs, they can experience weight gain. The rates of weight gain after stopping Wegovy vary depending on the individual, and available data suggests that with some individuals, weight gain will come back earlier, and with others, it may come later.

  • What information does PlushCare need for a prior authorization?

You will need to know the name, dosage, and how often you take your prescribed medication. It is important to know your insurance RX number, BIN number, PCN number, Insurance identification number and your address listed with your insurance provider. Our information will need to match with your insurance provider, if information does not match their records this can lead to further complications and delay receiving your medication.

This information is often listed on your insurance card. If you have any trouble finding this information or need to confirm information on file with your insurance, please contact your insurance before filling out our PA form.

  • What information do I need for my prior authorization?

If applicable, please list the other medications you previously tried for your specific condition. You will want to list the dates you took these medications and why these medications did not work for you. If you have specific lab results that support your need for the medication, you can also send that to our prior authorization department when filling out your prior authorization Form.  If you are applying for weight loss medication, please provide documentation / receipts from any earlier diet/exercise programs.

  • How do I start the prior authorization process?

To process a prior authorization (PA), you must be prescribed medication from your PlushCare doctor.

If your insurance requires a prior authorization, your pharmacist will tell you, you need a PA. If you are told that you need a prior authorization for the medication, please fill out our google form: https://forms.gle/KHry5fdh3z2X5bd86.

Once you complete this form, you will receive an email confirmation. Our prior authorization department can take 1-3 business days to collect the necessary information to send a PA to your insurance. An email confirmation will be sent to you when your prior authorization has been submitted to your insurance company.

  • How long does the prior authorization process take?

Prior authorization decisions and wait time are solely based on the insurance provider. On average, prior authorizations can take 7-14 business days before determination. We check for updates from your insurance company every 7 days. You will receive an email when there is an update with your prior authorization.  You may also get an email that more information is needed if your insurance information has changed, address has changed, or other information is needed to process your PA.

  • While I wait for my prior authorization, what can I do?

If you are applying for weight loss medications, you should schedule a chat with a Health Coach (free with membership). Our PlushCare Health Coaches work as a team with your doctor to ensure you meet your health goals. Sustained lifestyle changes are the main drivers of weight loss even when using medications. If you are applying for weight loss medications, some insurance companies require that you are enrolled in a comprehensive diet and exercise program with documentation of your monthly visits while on the medications. Our health coaches and doctors work together to provide you with this comprehensive diet and exercise program.

In between your doctor's visits, your coach is here to help you achieve the diet and exercise goals needed to sustain weight loss and become healthier long term!

About weight loss medications

Weight loss medications are often prescribed to help with chronic weight management.

Many Americans struggle with obesity and being overweight. According to the CDC, 42% of adults in the United States have obesity, defined as a body mass index (or BMI) over 30 kg/meter squared. This has been termed the obesity epidemic.

Over the past several years, prescription weight loss drugs have been developed and approved by the FDA to help fight the obesity epidemic. These weight loss medicines are approved for use in conjunction with healthy eating and exercise to promote weight loss in those who suffer from being overweight or obese.

Some weight loss medications work by either reducing appetite and/or increasing metabolism. Reducing one's appetite reduces the amount of food intake, which can result in weight loss. Increasing one's metabolism can help burn energy quicker and more efficiently, thereby allowing for weight loss.

Some prescription medications allow people to lose weight by activating chemicals in the brain that control hunger and cravings. By reducing the urge to eat or reducing cravings for foods high in fat, sugar, and starch, these drugs can help you lose body fat.

Some anti-obesity medications work mechanically; that is, they work within your gastrointestinal system to eliminate the absorption of  fat or increase feelings of fullness, leading to an overall reduction in energy absorbed, food taken in, and weight gain.

Types of weight loss medications available online

There are three main categories of weight loss medications: those that have been FDA approved for weight loss, type-2 diabetes medications, and other medications that don't fall into those categories.

  • FDA-approved for weight loss

A number of weight loss medicines are approved by the U.S. Food and Drug Administration (FDA) for the treatment of overweight or obesity. Prescription weight loss medicines can only be prescribed by a doctor after a qualifying evaluation.

Prescription weight loss medicines can help people lose weight, but it is essential that the medications are used in conjunction with healthy eating, physical activity, and behavior modification, as medication usage without such changes are often ineffective.

The following medications are FDA-approved for weight management:

  • Type-2 diabetes medication

Certain type-2 diabetes medications are not FDA-approved for weight loss, but may be prescribed off-label for weight loss, as they have shown to be effective in weight management.

For type-2 diabetes patients, these medications are sometimes taken alongside other type-2 diabetes medications, like metformin, to increase efficacy.

  • Other

FDA-approved to treat depression and for smoking cessation, bupropion is also sometimes prescribed off-label as a weight loss drug.

How weight loss medications work

Different weight loss medications have different mechanisms of action. Learn more about how the following commonly prescribed weight loss medications function:

  • Saxenda (liraglutide)

Saxenda (liraglutide) is a GLP-1 receptor agonist drug approved for the treatment of obesity and is administered as a once-daily subcutaneous injection. The GLP-1 agonist group of drugs works for weight loss by increasing fullness sensations in the stomach and slowing gastric emptying, resulting in weight loss. Liraglutide may be used in people with or without diabetes.

    Wegovy (semaglutide)

Wegovy (semaglutide) is another GLP-1 agonist drug. Administered as a once-weekly subcutaneous injection, it has a proven effect on weight reduction. Wegovy recently became FDA-approved to treat obesity in the absence of diabetes, as of June 2021.

    Alli, Xenical (orlistat)

Orlistat (trade names Alli, Xenical) inhibits the activity of an enzyme called lipase, which helps breakdown fat for absorption by the body. Orlistat therefore helps to block fat absorption. Orlistat is safe and effective for weight loss but is less well-tolerated due to its frequent gastrointestinal side effects.

    Contrave (bupropion/ naltrexone)

Contrave (bupropion/naltrexone) inhibits dopamine and norepinephrine reuptake in the brain and blocks opiate receptors. The end result of bupropion/naltrexone's activity in the brain is a reduction in appetite and food cravings. Concerns related to this drug include elevated blood pressure, lowering of the seizure threshold in the brain, as well as headaches and nausea.

    Plenity

Plenity is a capsule technically classified as a medical device rather than a drug, as it is not absorbed into the bloodstream. Approved in 2019 but not yet widely available, Plenity works by expanding in the stomach along with consuming 16 oz of water. Plenity is composed of cellulose and citric acid and forms a gel matrix after it is activated by water. Plenity does not absorb into your bloodstream. Plenity is able to be used for the management of excess weight in adults with a BMI of 25 to 40. This is a big advantage, as other weight loss medicines are generally approved for use only in those with a BMI over 27.

    Mounjaro (tirzepatide)

Mounjaro (tirzepatide) was recently FDA approved for the treatment of type 2 diabetes, but was also found to result in significant weight loss. It can be used as an off-label drug to reduce weight as well in patients with type 2 diabetes.

    Victoza (liraglutide)

Victoza is a liraglutide injection that lowers blood sugar by telling your body to release more insulin after eating. It also can help make you feel fuller for longer. A type-2 diabetes drug, Victoza also helps with losing weight.

    Ozempic (semaglutide)

Similar to Wegovy, Ozempic is another GLP-1 agonist drug consisting of semaglutide. It is only FDA-approved for to treat type-2 diabetes, although it can result in weight loss as well.

    Trulicity (dulaglutide)

Trulicity (dulaglutide) is also a GLP-1 agonist used to treat type-2 diabetes. It works by slowing digestion and lowering blood sugar by telling your body to release more insulin after eating.

    Wellbutrin (bupropion)

Similar to Contrave, Wellbutrin consists of bupropion, which inhibits dopamine and norepinephrine reuptake in the brain. This may result in a reduction in appetite and food cravings.
