To obtain a doctor's note from PlushCare, schedule an appointment with one of our online doctors. During your video or phone appointment, the doctor will assess your symptoms, medical history, and current medications. If medically necessary, they will issue a doctor's note.
Our physicians can provide various documents, such as work or school excuses, as appropriate. Please discuss your need for a work note with your PlushCare physician during your appointment for quick processing. In some instances, we can assist with more complex forms like FMLA or Disability forms, which may require additional appointments for thorough evaluation and review of medical records. Our doctors aim to provide the best care alongside long-term leave and other accommodations, which may sometimes necessitate further tests or an in-person assessment.
