What is alprostadil?

Alprostadil is a prescription medication typically prescribed to adults to treat erectile dysfunction (ED).1, 2 It comes in two forms: as a powder mixed with a provided liquid that is injected into the penis, and as a suppository or small pellet that is placed within the opening of the urethra (known as transurethral alprostadil).1, 2 Injectable alprostadil is sold under the brand names of Caverject, Edex, and Prostin VR, while the brand name of the alprostadil suppository is Muse.1, 3

How does alprostadil work?

Alprostadil is a type of medication called a vasodilator.1, 2 Vasodilators work by opening up the blood vessels by relaxing their walls and preventing them from contracting.4 In the case of alprostadil, the injection into the penis dilates the blood vessels that allow blood to flow in and erection to occur.1, 2

 

Erections typically occur within 5-10 minutes of administering an injection of alprostadil and within 5-20 minutes of inserting the suppository, and erections should last 30 minutes to an hour with each use and may persist even after ejaculation occurs.1, 3

 

Research has indicated that the injectable form of alprostadil tends to be more effective than the suppository form when treating erectile dysfunction.3, 5 About 80% of men can obtain and sustain an erection that is firm enough to have sexual intercourse when using injectable alprostadil.2, 3

Uses of alprostadil

The most common use of alprostadil is to diagnose and treat erectile dysfunction (ED) in adult men and people assigned male at birth.1, 2, 3 However, it may also be used to treat congenital heart defects in infants prior to heart surgery, and clinical trials are currently exploring the use of topical alprostadil in treating female sexual arousal disorder (FSAD).5

Alprostadil side effects

Like all medications, alprostadil can come with unwanted side effects alongside the desired ones. If you experience any uncomfortable or lasting reactions to injectable or transurethral alprostadil, you should talk to your doctor as soon as possible.

Common side effects

Bleeding or bruising at the point of injection or insertion is the most common side effect experienced by people who use alprostadil.1 When using transurethral alprostadil, you may experience some pain in the urethral opening, penis, testicles, legs, or perineum as well as redness, a rash, low blood pressure or prolonged erection (priapism).1, 4 Injectable alprostadil, meanwhile, may cause priapism, high or low blood pressure, swelling, rash, infection, or dizziness.1, 4

Rare but serious side effects

In rare cases, serious side effects can occur from taking alprostadil. If you experience signs of serious allergic reaction, have an erection lasting more than 4 hours, accidentally break the needle within your penis, or notice swelling of the veins in your legs, you should seek medical attention immediately.1

Are there any risks associated with alprostadil?

Using alprostadil does have some risks associated with it, and those risks differ based on whether you are using the injectable form or the suppository.2 The suppository, known as transurethral alprostadil, may cause urethral scarring, dizziness or fainting, a prolonged erection (known as priapism), pain, hypotension, and more. Alprostadil injections, meanwhile, can cause hypotension or hypertension, priapism, dizziness, swelling of or rash on the penis, bruising, and in rare cases, infection. 

 

Before starting on alprostadil, it is important to discuss all risks with your medical provider to create a safe and personalized treatment plan. 

How long does alprostadil last?

An erection caused by alprostadil should last between half an hour and 60 minutes.1, 3 It is important to note that your erection may persist beyond ejaculation. 

What should I do if I experience side effects from alprostadil?

If you experience side effects after taking injectable or transurethral alprostadil, you should contact your doctor as soon as possible, especially if they do not go away.1 In the case of emergency, such as an erection lasting more than 4 hours, needle breakage within the penis, or swollen veins in your legs, seek medical attention immediately.1

Can alprostadil be used with other erectile dysfunction medications?

You should never take other ED drugs at the same time as alprostadil, including Viagra or similar medications.2 Mixing erectile dysfunction prescriptions can potentially be toxic. 

Who should not use alprostadil?

Alprostadil should not be used if you are not diagnosed with erectile dysfunction, nor should it be taken in larger quantities or more frequently than instructed by your doctor and pharmacist.2 Improper use may cause permanent damage to the penis. You should also not use alprostadil without a condom if your partner is pregnant or planning to become pregnant, as it is currently unknown if the medication can harm unborn babies.1

 

If you have a history of priapism (prolonged erections), a penile implant, or any conditions that impact your penis, you should discuss your medical history with your doctor prior to taking alprostadil.2
